Integrated Care Support Services

The Pentathlon programme can be led by community members who aren't necessarily health professionals, as well as health and wellbeing coaches and similar roles found in primary care. This flexibility allows us to reach individuals or groups who might not typically engage with healthcare providers due perhaps to trust issues. We offer a fresh avenue of connection. Often, these are individuals from ethnic minorities and less affluent areas who experience disparities in their healthcare access, outcomes, and experiences.
 
By empowering our communities through this programme, our aim is to establish stronger, enduring relationships between local communities and healthcare professionals. This initiative targets existing health inequalities, enhancing community access to healthcare and improving their overall experiences and outcomes.

Pentathlon is a prime example of collaboration between health entities, local authorities, and VCSE (Voluntary, Community, and Social Enterprise) organisations. It aligns seamlessly with priorities outlined in the NHS Long Term Plan, the Core20+5 strategy, and joint forward plans. This programme champions prevention and self-management of long-term conditions, empowering communities to take charge of their health and wellbeing.
